Six Stages of a Quality Action Project

  1. Feasibility/Meeting with Executive Council. a) Assemble your team and fill out the top section of the Quality Action Project Worksheet together, then b) present your project to the Executive Council [schedule a meeting]. You may consult with QPT or a QPT member as needed.
  2. Complete the QAP Worksheet. Work through the rest of the worksheet with your team. Consult with the QPT or a QPT member as needed. Utilize Quality Tools as you analyze your processes to find where they can be improved.
  3. QPT Evaluation. Schedule a meeting with QPT to evaluate your plan and give feedback.
  4. Followup with the Executive Council (if required by QPT. If your project requires funding or other resources, schedule a meeting with the Executive Council. Be prepared to initiate the 3-P's Process as needed.
  5. Plan Execution and Periodic Updates. As you put your plan into action, schedule a progress report with QPT once every 6 months. Fill out duplicates of the "Updates" section of your QAP Worksheet each time.
  6. Completion and Retirement. Fill out the Completion Report and present it to QPT and the Administrative Cabinet [schedule a meeting].
