All forms (see below) must be turned in to the athletic office with proper signatures before you can practice. They must be submitted every year!
- ALL Athletes must read the
NCAA Insurance Memo. Get MBBC Insurance.
- ALL Athletes need Athletic Insurance Form signed by a parent. You will not be allowed to practice until this information is completed. Parent's signatures are required in the insurance information section.*
- ALL Athletes need Copy of Insurance Card on file - it is re-copied each year
- 1st Year Athletes need Completed Medical Forms turned in to the Admissions Office (this is part of your admissions file, please send this part to Admissions and not Athletics)
- 2nd and 4th Year athletes need Athletic Medical History done with a health professional--Mrs. Muse has time slots if desired.
- 3rd Year athletes need Physical Examination.
